New Patient Registration

You will need to complete a Registration Pack (available from reception). This should be completed and handed in to reception.  Please complete all fields on the registration forms as any errors may cause a delay in registration.

All patients ages 16 and above who apply to register with the practice must attend a new patient health check with the healthcare assistant. This is an extremely important health check as it can identify many potential problems and introduce you to the practice. 

Change of Personal Details

If you change your name, address, or telephone number please complete a 'Change of Details' form and hand into either Bedwell Medical Centre or Roebuck Surgery and your medical record will be updated accordingly. 

If you move out of the practice area, please advise us and ensure you register with a GP within the area you are moving to, as soon as possible.

We currently register patients who live in all areas of Stevenage, including Great Ashby.

Temporary Registration

If you are taken poorly when you are away from your home and staying in a different catchment area, for example staying with family members, staying with friends or working away from home and require medical treatment you can register as a ‘temporary patient’. A temporary patient must be staying in that ‘temporary area’ for longer than 24 hours but less than 3 months.

Temporary patients will still remain registered with their permanent GP, but can see a temporary GP for up to three months.

To register as a temporary patient simply contact the surgery for more details. The practice will not accept you as a temporary patient if you are residing within the catchment / local surrounding area.
