Adult ADHD referrals:

Below are the four forms that need to be completed by patients aged 18+ who have been referred for a possible ADHD diagnosis by a GP. Please complete all 3 forms and return to the surgery for the attention of the 'Medical Secretaries' - you must have an appointment with a GP to refer you. The team will then complete your referral and send you a text message to confirm referral has been sent off (electronically). You will then need to await contact from the Mental Health Team.
